registering as a patient

On registration you will be asked to make an appointment for a health check with a member of the nursing staff.  Please inform the receptionist if you are taking any regular medication as you will also be required to see a doctor.

You will also be asked to fill in a New Patient Questionnaire which are available from reception or you can download a copy of the Questionnaire by clicking the link below:
